Our office understands the value of Insurance benefits to our
patients. We will fill out and file your insurance forms at no
charge. We will also be glad to represent your interests with
your insurance company. Our goal is to make sure you receive the maximum benefit from your policy. We render services directly to the patient and they are responsible to us for payment. Your Insurance company will then pay you directly. |

in performing your needed treatment. Our goal is not to let expense prevent you from benefiting from quality care you need and desire. To facilitate this goal we have developed the following financial options: |

We are happy to offer a 5% pre-payment courtesy for all treatment in excess of $125 paid in full prior to treatment. |     |

For your convenience we accept payment by Master Card, Visa, American Express, and Discover. We offer a 3% pre-payment courtesy for all treatment in excess of $125 paid in full by credit card prior to treatment. | |

For patients who desire a monthly payment plan, we have made arrangements with Care Credit. There are no application fees, No down Payment, and the loan can be interest free. Same day credit approval is available. | |
